paper applications must be submitted no later than 16 days prior to the
desired effective date. This will provide sufficient time to enroll
members by their effective date, issue ID cards in a timely fashion, and
update systems for proper use of services.
We strongly suggest that all individual applications be submitted through
our online enrollment channel, AHNJ4U.com.
Online submission is the most efficient and accurate method for enrolling
individual members – ensuring proper payment allocation and ease of
tracking.
