The Individual Horizon Centurion Dental Program offers reduced fees to
members when they obtain eligible dental services from a participating dentist
in the Horizon PPO Network. Also available is the Horizon Individual DMO
Dental Plan. With this option members can access providers from
the Horizon Dental Choice Network and receive benefits according to the fee
schedule.
